Your health is very important to us! Please download this form, read it carefully, fill it out, sign and date where needed, and bring it to your first appointment.

24-hour Cancellation Notice:
If you must cancel or reschedule your appointment, 24-hr notice is required to avoid being charged for your full service. |
Late Policy:
Due to time constraints, if you are late for your appointment, your session will end at the originally scheduled time. Full payment is required. If we are late for your appointment, you may choose to end at the scheduled time (price will be adjusted) or receive the completed service for full-price. |
Informed Consent:
We want to be in partnership with you as you return to or maintain your state of health. In order to do that, we will be taking a thorough health history so that we can tailor your treatment plan to your individual needs. We will not perform any treatment or technique without your informed consent. |
Client Confidentiality:
Your treatment is considered a health care treatment and falls under the HIPAA laws. As such, all health history information, treatment plans, treatment logs, and use of services will be kept confidential unless you give signed permission to share your information with another health care professional. |
Accepted Payment:
Full Payment is required at the time of treatment. We accept cash, check, or charge. You may also pay for your service online using a charge card or Paypal. Fee for a returned check is $30.00. |
Ongoing Education:
We are committed to expanding our knowledge in the field of massage therapy. Our therapists are members of AMTA (American Massage Therapy Association.) They are also pursuing national certification and state licensing. In order to maintain certification, we are required to take Continuing Education Units. |
